Indiana Residents

In accordance with Indiana law, consumers have the right to:

  • Request access to their health care records held by this practice, subject to applicable state and federal regulations.

  • Contact the applicable licensing or disciplinary authority to report concerns or complaints regarding services provided by this practice.

  • File a consumer complaint related to privacy, record access, or professional conduct through the appropriate Indiana state agency.

Illinois Residents

In accordance with Illinois law, consumers have the right to:

  • Request access to their health care records held by this practice, subject to applicable state and federal regulations.  Requests must be submitted in writing and will be processed within the timeframes required by law.

  • Contact the applicable licensing or disciplinary authority (Illinois Department of Financial and Professional Regulation https://idfpr.illinois.gov/) to report concerns or complaints regarding services provided by this practice.

  • File a consumer complaint related to privacy, record access, or professional conduct through the appropriate Indiana state agency.

Texas Residents

In accordance with Texas law, consumers have the right to:

  • Request access to their health care records maintained by this practice, in accordance with applicable state and federal laws. Requests must be submitted in writing and will be processed within the timeframes required by law.

  • Contact the appropriate licensing or disciplinary authority (Texas Behavioral Health Executive Council https://bhec.texas.gov/) if they have concerns regarding the professional conduct of this practice or its providers.

  • File a consumer complaint regarding privacy practices or the handling of protected health information as permitted under Texas Health & Safety Code §181.103. Complaints may be submitted to the appropriate state regulatory agency.
